📋 To ensure a smooth process and respect for our Injectors’ time, we kindly request that all intake forms be completed at least 24 hours prior to your initial appointment. If your forms remain incomplete and show as “Pending,” we will be unable to proceed with your appointment, and the appointment will be canceled if the forms are not completed within 24 hours. We will happily remind you and notify you if your appointment does get cancelled.
💳 Credit card information is required to complete the intake forms and for them to be submitted for review by our injectors. A credit card on file is necessary for our No-Show and Last-Minute Cancellation policy; however, you will not be charged unless you cancel within 24 hours of your appointment or no show.

ALL NEW patients must book a virtual or in-person pre-assessment appointment PRIOR to booking treatment as all treatment plans need to be approved by our Medical Director.
If you book otherwise, we will contact you to re-book your appointment.
Wrinkle Relaxers (Botox/Dysport) Treatment
Neuromodulators (such as: Botox and Dysport) are FDA approved injectable treatment that can improve the appearance of dynamic expression lines and wrinkles by causing relaxation of the facial muscles (for example, forehead lines, frown lines, crow's feet etc.).
Dermal Filler Treatment
Dermal fillers are injectable sterile gels made up of Hyaluronic Acid (HA) and/or Calcium hydroxylapatite (CaHA), that are used to restore volume to the skin, improve facial lines, wrinkles and folds as well as enhance facial contours.
Skin Rejuvenation Treatments
Skin Hydration (SkinVive) is a facial rejuvenation treatment designed to add hydration and glow directly into the skin.
Collagen Stimulators (RADIESSE) are an injectable gel filler made up of Calcium Hydroxylapatite (CaHA). Injected into the skin layer, it will aide in smoothing folds & lines as well as stimulate collagen & elastin production which naturally breaks down and slows in production as we age.
VAMP (Vitamins, Amino Acids, Minerals and PDRN) is a topical, sterile, bio-revitalizing treatment enriched with 11 Vitamins, 23 Amino Acids, 6 Minerals, Polydeoxyribonucleotides (PDRN) and Hyauronic Acid to aide in skin rejuvenation, and revitalization. It helps to improve hyperpigmentation, skin tonicity, tightens pores, smooth wrinkles, and restore skin's glow and hydration. Platelet Rich Fibrin (PRF) Treatment
Platelet Rich Fibrin (PRF) is a new generation of Platelet-rich Plasma (PRP) that achieves longer lasting & more effective end results.
The procedure uses your own blood to collect a ‘liquid gold’ concentrate of platelets, fibrin, stem cells and white blood cells, which helps achieve wound healing, collagen production and improved elasticity.
The Platelet-Rich Fibrin Gel technique uses the latest generation of Smart Blood Concentrates to generate a complex Fibrin Matrix rich in Platelets, Leukocytes and Mesenchymal stem cells.
Fat Dissolving Treatment
Fat Dissolving Injections (deoxycholic acid) injections are indicated for improvement in the appearance of moderate to severe convexity or fullness associated with submental fat, also called ‘double chin’, in adults.
